Grades 1-5 Application for Admission

The submission of a completed application form (including General Information and Parent Questionnaire) and the $90 application fee begins the admission process.  Following the receipt of the application and fee, we will contact you to schedule your child's classroom visit.  Applications must be received by January 15th for first round consideration.  All other materials are due by March 1st.

We also require an official school transcript.  Please print the Transcript Release Form (PDF) and submit it to the Registrar at your current school.  The transcript should be mailed directly to Providence Day School by March 1st.

Within five business days of submitting your application and fee, you will have access to a checklist that will allow you to see all information that has been submitted and received.
