Grades 6-12 Application for Admission

The submission of a completed application form (including General Information, Parent, and Student Questionnaires) and the required $90 fee begins the admission process.  Applications must be received by January 15th for first round consideration.  All other materials are due by March 1st.

We also require an official school transcript which should include the first semester grades from the current school year.  Please print the Transcript Release Form (PDF) and submit to the Registrar at your current school.  The transcript should be mailed directly to Providence Day School by March 1st.

Within five business days of submitting your application and fee, you will have access to a checklist that will allow you to see all information that has been submitted and received.
